The LSCP's Serious Incidents & Reviews Subgroup is chaired by Avis Williams-McKoy (Designated Nurse for Safeguarding, Lambeth ICS) and deputy chair is Brenda Mcinerney, Acting Director of Practice, Performance and Partnership, Lambeth Children's Social Care).
